As much as I enjoy a good Five Guys burger, In & Out wins hands down (it's my Southern Cali upbringing). Best thing about In & Out is that there's countless things that aren't on the menu that you'd never know they served unless you just knew they served them...did you know you can get a grilled cheese at In & Out? :)
If I have to pick one I'll go with Five Guys. My second choice would be White Castle. It's been a long time since I've had White Castle. My mom served White Castle for my sixteenth birthday party. I miss those little burgers so much.
Ditto on Ray's. I feel like if they attempted fries, they'd knock them out of the park. But I love the confidence of not serving fries, it's like "no, you are going to like our burgers so shut up and enjoy it."
i'm going to join the chorus of five guys. that stuff is tatsy. they finally opened one in austin and i took all my texan friends there and they all felt like they had died and gone to heaven! plus they give you a bag full of fries. cmon!
I don't understand how five guys doesn't serve milk shakes. Seems obvious to me guys.
Theres also a place in Durham, NC called Cook Out. They have about 40 different flavors of milk shakes and you can get a cook out tray (burger or chicken sandwich plus two sides and a drink, sides include: french fries, hush puppies, chicken nuggets, another burger, and about four other ridiculous options) for about $3.50. Awesome.

I do have to claim some local pride and say that Tonic makes a damn good guacamole/cheddar burger, though.

Good Stuff Eatery on The Hill. I heart Spike!

Of the options mentioned, 5 Guys wins in a walk. Elsewhere, Booche's takes the title.

Ray's Hell Burger in Rosslyn. They give you a paper towels instead of napkins.

OOOO! Totally seconding the Skellar. And as totally suburban as it sounds, Cheesecake Factory actually makes damn good burgers.
